Patient Care Manager, Hospital at Home Program

A wonderful opportunity to be on the cutting edge of starting up a new program for a large and well established hospital based agency.  A Bachelors prepared RN with both acute and home care experience is preferred.  Must know Medicare and be able to do the initial assessments.  ICU or ER background helpful.
